- Tracking how often staff members remind patients to get screenings
What can your team do to ensure that members and patients get their necessary screenings? What else could your team do to coach and mentor HeatlthConnect super-users?
This eye care unit-based team in Redwood City takes full advantage of Kaiser Permanente’s “under one roof” care system and HealthConnect to help patients get the cancer screenings they need. As Dr. Anthony Liu says, many patients only come to eye appointments and may not visit a primary care clinic. Dee Dee Crick, a vision service assistant, is the self-proclaimed “prompt police.” She coaches her teammates on how to use the prompts in the electronic health record to book cancer screening for patients.
Thanks to HealthConnect and Kaiser Permanente’s integrated care model, this eye-care team has the tools it needs to make sure patients get overdue preventive care screenings.
